*Reports to:* DON/Administrator
*Job Classification*: Hourly, Non-Exempt
*Supervisory Duties*: Ensures that all the needs are met for the clients as well as supervises staff that is on the shift at the Facility.
*Minimum Qualifications*: Must be a graduate with an accredited LPN school.
Must be licensed with the State of Minnesota as a LPN, and must maintain current and unencumbered licensure.

* Because the Facility performs service 24 hours a day, 365 days a year, staff will be required to work the shifts/ schedules for which they are hired and will include every other week-end.
Employee must be willing to work part –time or full time, and during half of the holidays recognized by the Facility.
* Employee must be willing to undergo on the job training before being certified to work with residents including background check.
* Employee must be flexible and willing to adapt to changing situations in a professional manner.

* LPN’s are required to give a 3 week notice if resigning from the position.
* Employee must pass background check prior to providing cares.
* Employee must work holidays and every other weekend.

* The employee actively participates and supports organizational initiatives *Responsibilities Include (but not limited to) the Following:* * Provide companionship and supervise resident activities * Must always treat clients with complete respect and cooperation-no exception * Assist with any needs of clients first * Works to assure facility is in compliance with federal state and local standards and regulations.
Is available for license surveys.
* Maintain client files and appropriate documents as required.
* Monitors clients on an ongoing basis to assure their needs can be met.
* Assures clients continue to live safely in their unit, monitors and recommends alternative care and services, if necessary.
* Inform personnel of changes in client’s needs and conditions.
* Assists UAPs and Caregivers in service delivery.
* Maintains communication with clients, families, and other staff.
* Responds to client concerns or complaints.
* Check for Doctors orders that may come in when RN is out.
RN must be notified within 1 hour of any new orders that are received on your shift.
* Monitors and follows thru with ordering medications and all medication renewals as well as any necessary follow-up with physicians regarding medications.
* Fills weekly pillboxes for medication administration by unlicensed staff and checks new medications from the pharmacy.
* Call to inform and obtain authorization from RN for any med changes.
* Call to notify RN of any new orders, faxed documents.
* Communicate significant changes to RN.
* Communicates to physicians any changes in client’s needs or conditions, as needed.
* Communicates and documents client data to staff as needed.
* Communicates in staff communication book any information that staff needs to be aware of.
* Complete any med setups as delegated by the RN.
* Complete any insulin draws as delegated by the RN.
* Complete changes on MAR per RN’s instructions.
* Destroys medications per policy and regulatory requirements.
* Prepare paperwork for any Dr.
visits.
* Chart appropriately on clients, assist with weekly charting on clients.
* Assist with bathing, dressing and grooming as needed.
* Perform accucheck and insulin administration as needed.
* Pass meds and comply with regs, policies and procedures.
* Assures clients maintain residency requirements, monitors and recommend alternative placement if necessary.
* Reviews client service agreements with RN, as needed.
* Documents and monitors and maintains client charts and nursing notes.
* Completes supervisory visits under the direction of the RN accordingly to regulation and policy.
* Completes post fall evaluation per policy and under the supervision of the RN.
* Assist to serve meals.
* Assist with dining room duties.
* Perform duties on “Shift Duty sheet” that pertain to you.
* Must be willing to attend on the job training sessions prior to being certified to work with clients, must be willing to have Orientate to Home Care if needed.
* To be in attendance for scheduled shifts or have found a suitable replacement to work for you.
* Attend 80% of monthly staff meeting and 100% of any required training.
* Assists with compliance in meeting regulations and is available for state licensure surveys.
* Perform all other responsibilities as assigned or delegated by the DON/Administrator or RN.
