Support the mission, vision and values through unequaled quality patient care, professional competence, collaboration, communication, innovation, accountability, ownership and provide support to the entire agency regardless of their primary office location.
Position Summary
Provides care-based education and mentoring for providers and staff, and aides in identifying opportunities for new and improved hospice involvement and integration into providers’ environment as it relates to both staff and patient.
Facilitates hospital referrals and admissions to include coordination of discharge from hospital to Hospice care.

The employee is frequently asked to drive, occasionally drive to unfamiliar destinations.
Qualifications
• Licensure as a Registered Nurse
• Five or more year experience in nurse leadership
• Knowledge of hospice services and business practices
Certifications, Licenses, Registrations Required
• Current state RN licensure in North Dakota and Minnesota
• Current drivers license and auto insurance
