*POSITION TITLE:* ACTIVITY AIDE *DEPARTMENT:* NURSING HOME (UPPER & LOWER LEVEL) AND SWING BED *SUPERVISED BY:* ACTIVITY DIRECTOR *SUPERVISES:* NONE *STATUS: NON-EXEMPT * *CLOSE WORKING * *RELATIONSHIPS WITH:* NURSING HOME RESIDENTS, NURSES, CNA’S, FAMILY MEMBERS *JOB SUMMARY* The Activity Aide will assist the Activity Director to carry out activity programming to meet each individual resident/patient needs/preferences based on the initial Activities Assessment (completed by the Activity Director) in the areas of physical, mental, psychosocial and spiritual.
*EXPERIENCE & SKILLS: * Experience in working with geriatric populations, including those with memory impairment.
*EDUCATION & TRAINING: * Must be attending High School or have a High School diploma.
*Required Licensure/Registration/Certification:* None.
*SPECIAL SKILLS: * 1.
Good verbal and written communication skills.

Able to be self-disciplined, work independently and follow scheduled plans.

*POSITION SUMMARY * *RESPONSIBILITIES: * 1.
Read and understand the written goals and approaches for each individual.
2.
Prepare, carry out and clean up large and small group and 1:1 activities.
3.
Assist the residents/patients during activities as needed.
4.
Observe resident’s/patient’s participation and responses during activities.
5.
Document daily each resident’s/patient’s participation and assistance level as well as his/her response to the activity.

Communicate verbally or in writing any changes or concerns with individual residents/patients to the Activity Director or immediate concerns to Nursing.
7.
Assist at meal times with passing liquids, plates, cover-ups and feeding (once trained).
8.
Assist residents/patients to and from activities and meals.
9.
Update activity white board daily.
10.
Chaperone out-of-facility scheduled events.
11.
Assist/respect other departments in the facility.
12.
Assist with putting up, taking down and storing seasonal decorations.
13.
Help care for resident’s/patient’s plants/flowers.
14.
Assist residents/patients with displaying cards, pictures, etc.
in rooms.
15.
Deliver and read mail to residents/patients as needed.
16.
Participate in and promote participation in facility-wide activities.
17.
Attend mandatory facility and department in-services and meetings.
18.
Ensure work areas and equipment are clean, sanitary and safe.
19.
Review, understand and follow through with facility policies and procedures.
20.
Maintain confidentiality of all resident/patient information.
21.
Must be able to cope with and respond respectfully to potential hostile and emotionally upset residents/patients, families and/or visitors.
22.
Must be able to work with frequent interruptions.
23.
Direct volunteers to their job duties and make them feel welcome.
*MENTAL DEMANDS:* 1.
Must be organized and able to complete assigned tasks during assigned shift.
2.
Minimal amount of intense concentration.
*PHYSICAL DEMANDS:* 1.
Sit, stand, bend, squat, kneel, reach, walk intermittently throughout the day.
2.
Lift and carry up to 30 pounds with good body mechanics.
3.
Able to push/pull a person in a wheelchair.
4.
Must have good fine motor and coordination skills to assist residents/patients with their activities as needed.
*SPECIAL DEMANDS:* 1.
Position requires enthusiasm, friendliness, and caring personalities.

Must be able to cooperate and work well with others.
3.
Must be able to show patience and tact with residents, families, visitors, and co- workers.
4.
Must be conscientious and respect patients and resident’s privacy and personal possessions.

Must practice good personal hygiene and hand washing.
6.
Must be calm and resourceful in emergency situations.
7.
Must be dependable in working shifts scheduled, and come in extra as needed.
*WORK ENVIRONMENT:* Works throughout well-lighted, well-ventilated facility.

Participate in Fire Safety Plan of Action.

Participate in Disaster Plan of Action.
3.
Participate in Universal Precautions for AIDS, Hepatitis and Tuberculosis.
4.
Follow facility established protocol for Isolation and Blood-borne Pathogens.
*CONFIDENTIALITY:* Frequent contact with residents and medical information in the normal performance of duties.
Confidentiality is essential.
Disclosure may affect internal and external relationships.
